Read carefully. This Release and Waiver of Liability must be accepted by a parent or legal guardian before a minor participates in any Pro. Dev. Unlimited ("PDU") program.
Basketball and athletic training involve inherent risks, including physical injury. I understand these risks and voluntarily allow the participant named at registration to take part in PDU sessions, clinics, tryouts, team practices, and games.
I confirm the participant is physically able to take part. In an emergency, I authorize PDU staff to obtain medical treatment for the participant, and I am responsible for any resulting medical costs. I will inform PDU of any medical conditions relevant to safe participation.
To the fullest extent permitted by law, I release and hold harmless PDU, its coaches, staff, and partner facilities from claims arising out of ordinary negligence connected with participation. This release does not apply to gross negligence or willful misconduct.
I consent to PDU photographing or recording the participant during activities and using such media for team and promotional purposes, unless I opt out in writing.
By registering the participant, I confirm I am the parent or legal guardian, I have read this Release and Waiver, and I agree to its terms.
